Online testing options?

Hi all,
We are looking for software solutions that can be used to create and administer tests.
Key requirements are the ability to (1) shuffle the order of the questions, (2) shuffle the order of multiple choice answers, and (3) the ability to use a timer for questions or segments of the test.
www.mytestcom.net is one vendor that can meet these requirements, and I would like to know if there are any other software solutions, whether hosted on a vendor’s site or something that can be installed on a company’s server.
Please let me know if you have any suggestions for solutions or comments about testing packages.
